        Everybody keeps referring to this stuff as "old" and "simple" yet I can't
        find a "textbook" answer.... Granted a book on the subject wouldn't
        find its way onto the best seller list....


D. W. Barron. Assemblers and Loaders, third edition. North-Holland,
1978.


This book contains a chapter on loaders and linkers.
