Have a look in
@article{Deussen,
AUTHOR = {P. Deussen and L. Wegener},
TITLE = {A bibliography of the van {W}ijngaarden grammars},
JOURNAL = {Bulletin of the European Association of
Theoretical Computer Science (ETACS)},
YEAR = {1978},
VOLUME = {6}

There are many, many articles and books that were published before 1978
on the topic. You might want to follow up the affix-grammar trail, many
authors quote more or less obscure papers on vW Grammars in their papers.
