by J. Craig Cleaveland and Robert C. Uzgalis
#4 in the Programming Languages Series

1977, ISBN 0-444-00199-9 pbk.
It is the only reference I know of on this topic that has been published
and made available in the USA. There are references to earlier works on
various related topics.
